He was predeceased by : his parents, Raymond Cook and Ethel Cook (Mahan); his wife Geneva Wagner; and his sister Phyllis Diver (Helen Walton). He is survived by : his daughters, Karen Crawford (David) of Ivel, KY, Becky Ludlow (Jim) of Livton, Karla Cook (Mark Fight) of English and Janice Logsdon of New Albany. He is also survived by 12 grandchildren, 21 great-grandchildren, 11 great-great-grandchildren, and several nieces and nephews.
