He was predeceased by: his parents, Wilbur Francis "Ike" Cook and Eldora Broshears Cook; his brother Stephen Earl Cook; and his sister-in-law Terry Merritt Young. He is survived by: his wife Donna Gale Merritt; his daughter Rev. Beth Ann Cook (Art); his brother-in-law Larry Merritt (Julie); his sisters, Frances Ann Smith and Karen Sue Helmbock; and his nieces and nephews, Jamie Madigan (Pat), Amiee Smith, Jodie Young Drone, Jeanie Young Ferguson (Andy), Stephen Helmbock, Joe Kolezar, Rob Kolezar and Jim Kolezar.
