He was predeceased by: his parents, Wilburn E., Sr. and Delia Marie Butts (Collins); his daughter Kimberly Tice; his siblings, W. E. Butts, Jr. (Iva Lois Butts) and Helen Kessler Ramberger; and his father-in-law Clarence Hazlett.
He is survived by: his wife Donna Hazlett; his mother-in-law Erma J. Hazlett; his aunt Janice Cain; and his extended family, Sharon, Angie, Dara, John III (Andrea) and Dallas Tice. He is also survived by several nieces and nephews.
