He was predeceased by: his mother Judith Ann Phillips (Perdue). He is survived by: his daughter Shae Phillips of Evansville, IN; his father Richard Phillips (Paulette) of Evansville, IN; his sister Shannon Phillips of Evansville, IN; his girlfriend Fallon Schmitt of Evansville, IN; and his grandchildren, Carter Myers, Caiden Myers and Grayson Myers.
