He was predeceased by: his daughter Lori Dale. He is survived by: his wife Annette Smith (Johnson); his sons, Chris (Kalah) and Craig Smith (Jill); his sister Bonnie McClure; and his stepsons, David, Joseph, Tim and Jason McMurray. He is also survived by many nieces and nephews.
