He is survived by: his wife Ginny; his children, Patrick Cook (Pamela) of Sammamish, WA, Paula Kepes (Frank) of Evansville, IN, Joseph Cook (Lisa) of Santa Claus, IN and Julia Zella (Curtis) of Horicon, WI; his grandchildren, Katherine Cook (Trevor Green), Daniel Cook, Sarah Cook, Alexander Kosiak (Christina), Christopher Kosiak, Olivia Cook and Martin Cook; his stepgrandchildren, Jacob Kepes and Hannah Kepes; his great grandchildren, Leona Kosiak and Vera Kosiak; his brothers, Michael (Juanamaria) of Columbia, MO, Gary (Monica) of Statesville, NC and Kevin (Pamela) of Whitehouse Station, NJ.; and his caregiver David.
