He was predeceased by: his father Garth Lively; and his brother Mike Lively. He is survived by: his wife Nancy Lively (Rueger) of Evansville, IN; his children, Lesley Dolan (Barry) of Ocala, FL and Grant Lively (Julie) of Evansville, IN; and his grandchildren, Mia, Wyatt and Reese.
