He was predeceased by: his parents, John Cook and Imogene Cook (Moore). He is survived by: his fiancée Barbara McCormick; his son John Anthony Cook (Devon); his stepchildren, Lisa Burdick (Jim), Brad McCormick (Andrea) and Brandyn McCormick; his grandchildren, Camden, Braylyn, Brilee, Makenna, Sofie and Porter; and his stepchild Shih Tzu Oreo. He is also survived by many nieces, nephews, extended family and friends.
