He was predeceased by: his parents, Oswell Cusick and Grace Cusick (Wetter); and his sisters, Jane Elsner and Marilyn Beverly. He is survived by: his daughters, Kim Goodwin (Andy) of Fort Wayne and Becky Wright (Steve Fanning) of Fishers, Indiana; his granddaughters, Madison and Bailey Goodwin of Fort Wayne, Indiana; his siblings, Glenn Cusick of Lisbon, Ohio and Shirley Thirion (Bob) of Lisbon, Ohio; and his niece Amy Goodwin. He is also survived by several nieces and nephews.

In lieu of flowers, contributions can be given to Solace Hospice Care, 280 Commercial Street, Roanoke, Indiana 46783.
