He was predeceased by: his parents, Arthur Bookout and Juanita Bookout (Weaver); his wife Carole Bucher; his siblings, Mary Fultz (Ed), Phyllis Jontz (Jon), Nancy Hunter (Dean) and Daniel Fultz; and his stepsiblings, Ed, Virginia, Helen, Betty, Dorothy, Nancy and David.
He is survived by: his daughter Suzanne Butz (Michael); his grandsons, Andrew Butz and Nicholas Butz (Casey); his great grandchild Charlotte Rose Butz; and his sister Janet Meyers. He is also survived by many nieces and nephews.
Visitation was held on Wednesday, September 16th 2026 from 9:00 AM to 12:00 PM at the D. O. McComb & Sons Funeral Homes - Pine Valley (1320 E Dupont Rd, Fort Wayne, IN 46825). A funeral service was held on Wednesday, September 16th 2026 from 12:00 PM to 1:00 PM and from 1:30 PM to 2:00 PM.
In lieu of flowers, memorial contributions may be made in Guy's honor to FWCS Foundation or St. Joseph United Methodist Church.
