He was predeceased by: his parents, Alexander Choinski and Mattie Choinski (Spencer); and his sister Ferome Castillo. He is survived by: his wife Lyn Choinski; his children, Shelly Choinski and Jerome J. Choinski, Jr.; his grandchildren, Hailey Webster and Jerome Choinski III; and his siblings, Frances Kleber (Steve), Alex Choinski (Mary) and Greg Leazier.
