He was predeceased by: his parents, Carol Cook and Darlo Cook; and his brothers, Darlo and Tony Cook. He is survived by: his siblings, Joy Miller (Dave), Cindy Anderson (Daniel), Dawn Gibson, Willis, Shane and Chad Cook; his daughter Amy Caudill; his granddaughters, Samantha Fredrickson (Ross), Jessica Caudill, Desirae Rider (Zach) and Victoria Mills; and his close friend Vickie Addison. He is also survived by many great-grandchildren, nieces and nephews.
