He was predeceased by: his daughter Allison "Ally" Jacobs. He is survived by: his father Steven Earl Jacobs (Shirley); his wife Michelle Jacobs; his children, Ashley Balcazar (Cesar), Nicholas "Nick" Jacobs (Tien) and Jordan Jacobs; his mother Candy Jacobs (Candy); and his brothers, Scott Jacobs (Cindy), Benjamin Jacobs (Courtney) and Peter Jacobs (Sarah). He is also survived by numerous nieces and nephews.
Visitation will be held on Thursday, August 28th 2025 from 5:00 PM to 8:00 PM and on Friday, August 29th 2025 from 11:00 AM to 1:00 PM at the FairHaven Funeral Home and Cremation Services (6557 N Clinton St, Fort Wayne, IN 46825). A funeral service will be held on Friday, August 29th 2025 at 1:00 PM at the same location.
In lieu of flowers, memorial contributions may be made to Cancer Services of NE Indiana.
