He was predeceased by: his parents, Bob Cook and Barbara Cook (Brown); and his wife Sally Ann Purdy. He is survived by: his siblings, Cindy (Jack Moore) of Franklin and Tim Cook of Greenwood; his aunt Bonnie; his uncle Dennis Brown of Noblesville; his niece Stacy Lanphier (Jamie); and his nephews, Greg Pierle (Kiana) and John Moore (Amy). He is also survived by eight great nieces and great nephews.
