He is survived by: his wife Mary Ann Isaac; his children, Chad Collyer, Shawn Collyer (Ashley Davis), Kevin Combs and Beth Paul; his grandchildren, Jackson, Maxwell Collyer, Joshua, Jesse and Haleigh Combs; and his caregivers, Amber, Paige and Melanie. He is also survived by several nieces and nephews.
