He was predeceased by: his parents, Dewey Whitaker and Lora Price Whitaker; and his wife Janet Lee (Kirby) of Somerset. He is survived by: his daughters, Terry Witzerman (Jim) of Greenwood, IN and Tammy Shackelford (John); his grandchildren, Amanda Harris, Marisa Witzerman, Jon Shackelford and Chelsey Holderman; and his great grandchildren, Aubrey Witzerman and Ayla Harris.

In lieu of flowers the family requests Memorial contributions be made to Franciscan Hospice Care, Franciscan Alliance Foundation - Central Indiana, 5255 E Stop 11 Road, suite 245, INDIANAPOLIS, IN 46237 in memory of Jerry Whitaker.
