He is survived by: his wife Carolyn Ann Hemmer Koontz of Hanover, Indiana; his children, Mark Koontz (Angela) of Madison, Indiana and Linda Ann King of Madison, Indiana; his grandchildren, Kara Bayne (Wes), Katelyn Carter (Wyatt), Jaydon Douglas Koontz and Dayton Owen King; his great grandchildren, Everly June Bayne, Lainey Claire Bayne, Colton Liam Carter and Alaina Lynn Carter; and his siblings, Duane D. Koontz of Hanover, Michael D. Koontz of Hanover, Indiana, Deborah L. Jacobson of Brooksburg, Donna M. Lytle of Madison and Marta D. Tingle of Madison, Indiana. He is also survived by several nieces, nephews.
