He was predeceased by: his parents, Frank Polak and Irene Polak Sr.; his former wife Linda Polak; his wife Judith Polak (Potocki); his son Robert Polak Jr.; and his brother Frank Polak Jr..

He is survived by: his children, Christopher Polak (Amanda), Michael Polak (Pam) and Tammy Sanders (Jay); and his grandchildren, Ashley, Samantha and Kristen Polak. He is also survived by other loving family members and friends.
