Underwood Sr. (Karen) and Tonja Mae Shafer (Mills); his grandchildren, Jarek Carter (Alexis) of San Antonio, Jillian Carter of Colorado Springs, Mikayla Allen of Indianapolis, Indiana, Kamrynn Wolchesky-Gully of Indianapolis, Indiana and Micah Allen of Indianapolis, Indiana; his great grandchild Tyrus Carter; and his siblings, Aaron Shafer of Jeffersonville, Amanda Shafer of Hartland, Camen Underwood of Noblesville and Josie Underwood of North Carolina.
