It is with great sadness that we announce the death of Jerry Cook Sr. of Indianapolis, Indiana, who passed away on November 6, 2023, at the age of 80, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Clarence Cook and Fern Cook; and his daughter Joni Cook. He is survived by: his wife Patricia Cook; his children, Jerry Cook, Jr. and Amber Cook; and his grandchildren, Noah Cook, Danielle Pope and Kayla Dixon.

Memorial contributions may be made to Riley Childrens Hospital.
