He was predeceased by: his grandparents, Tom, Mary Lou Phillips, Bill and Mildred LaGrange. He is survived by: his parents, Pam Frederiksen (Terry) and John Phillips (Nancy); his brother Zach Phillips (Shelby); his stepbrothers, Matthew Frederiksen (Amanda) and Michael Frederiksen; and his sibling's children, Deacon, Elsie, Hannah, Danika and Rayna. He is also survived by many aunts, uncles and cousins, as well as many close friends.
In honor of Caleb's life and legacy, the family kindly requests that, in lieu of flowers, memorial donations may be made to the Epilepsy Foundation Indiana, an organization committed to supporting those affected by epilepsy through education, advocacy, and community programs.
