It is with great sadness that we announce the death of Samuel Milton Hazlett III of Indianapolis, Indiana, who passed away on January 11, 2026, at the age of 81, leaving to mourn family and friends. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Samuel Milton Hazlett Jr. and Ethel Jane Hazlett; and his sister Judy Hazlett. He is survived by: his caregiver Suzanne Brown; his wife Margaret Hazlett (Moore); his children, Anne Hazlett and John Hazlett; his daughter-in-law Sarah Evans; his brother William Hazlett; and his grandchildren, Levon, Axl, Kyler, Christiana and Dayne. He is also survived by the many friends, family members.
