He was predeceased by: his brother Gary Blaize (Peg). He is survived by: his children, Lindsay Moll (Jason) of Montgomery, IN, Lacey Blaize of Jasper, IN and Luke Blaize (Madelyn) of Jasper, IN; and his grandchildren, Keegan Thomas, Karisma Moll, Devin and Owen Blaize. He is also survived by two nieces and one nephew.
