With heavy hearts, we announce the death of Jay Anthony Middleton of Kendallville, Indiana, born in Fort Wayne, Indiana, who passed away on March 31, 2024 at the age of 71. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.
He is survived by: his son Jesse Middleton (Ariel) of Pleasant Lake; his grandchildren, Braxton Middleton and Cadie Middleton; his stepgrandchildren, Haylee, Damien and Maddux; his siblings, Mark R. Middleton (Gina) of Fort Wayne, Billie Jo Rupert of Auburn, Jodie Evans (John) of Auburn and Connie Middleton of South Bend; and his close friend Lori Middleton of Kendallville. He is also survived by several nieces and nephews; and his caregiver.
