He was predeceased by: his parents, Everett Horner and Naomi. He is survived by: his wife Jeana Horner; his children, Todd Horner (Jess), Tara Ault (Randy), Jay Chapman, Jeremy Chapman (Aaron) and Jessica O'Neil (Zach); his grandchildren, Tyler Ault (Hannah), Collin Ault (Emily), Nate Horner, Payton Chapman, Alania, Aaiden Tutrow and Anora O'Neil; his siblings, Butch Horner and Zelma Kinney; and his pet Reggie. He is also survived by many nieces and nephews.

Memorial contributions may be made to the American Lung Association A celebration of Jack's life will be held in the spring.
