He was predeceased by : his parents, Donald Tolen and Mary Jane Tolen (Peak); and his siblings, Vernon Martin and Nickie Scott. He is survived by : his siblings, Roberta Pearce of Ft. Wayne and Frank Warren of Seymour. He is also survived by three grandchildren.

The family asks that in lieu of flowers, please make memorial contributions to the American Cancer Society.
