He was predeceased by: his parents, Dennis D. Cook and Janet Cook Yater (Smith); his son Ryan Cavanaugh Cook; his brother Michael Cook; and his mother-in-law Ginny "Woman" Cavanaugh.
He is survived by: his wife Mary Cook of Lafayette; his children, Lacey Cook (Cass) of Indianapolis, Korbin Uemura (Micah) of Indianapolis and Alexis Stwalley of Lafayette; his sister Shannon Glass (Tony) of Avon; and his grandchildren, Oden, Anya Hunter and Novi Uemura.
Memorial donations may be made to St. Jude Children's Research Hospital in loving memory of Cookie.
