He was predeceased by: his grandparents, Dewey Caudell, Ollie Caudell, Dave Chalk and Betty Chalk; and his uncle Delbert Caudell.
He is survived by: his parents, Ivan Chalk, Sr. and Freda Caudell Chalk; his fiancé Carrie Faulkner of Lawrenceburg, IN; his children, Noah Chalk of Mason, OH, Olivia Chalk of Lawrenceburg, IN and Gracie Chalk of Bright, IN; and his sister Jen Tuck (Bob) of Yorktown, IN. He is also survived by 4 aunts; 6 uncles; and numerous nieces, nephews, cousins and friends.
