He was predeceased by : his mother Linda D. Mayfield (Elliott); and his grandchild Baby Bushy. He is survived by : his father Gary L. Mayfield of Linton; his daughter Chloie D. Mayfield of Linton; his siblings, Leslie D. Cliver (Jon) of Dugger and Daniel R. Mayfield (Lisa) of Saginaw, Michigan; and his grandchildren, Willow, Kyler Bushy and Maizlee.

In lieu of flowers, memorial donations may be made to Good Samaritan Home Health Care of Vincennes or ViaQuest Hospice, through the funeral home.
