He is survived by: his wife Judith "Judie" Elaine Shank Wortham of Madison, Indiana; his nieces, Kendra Wortham Marquez of Fresno and Leandra Wortham; and his siblings-in-law, Joellen Shank Ammons (Jerry) and James L. Shank (Connie).
