She was predeceased by: her parents, Jeremy Perez and Kate Perez; her grandparents, Jose, Amy Perez, John and Jacque Sickert; her great-aunt Elizabeth Dodson (Chris); her uncles, Tracy Lombardo (Chris), Jonathan Perez (Katie) and Nick Sickert (Jenny); and her cousins, Tyler, Trevor, Lizzie, Lainey, Eliana, Lincoln, Harrison, Gabriel and Elsie.
She is survived by: her sister Evelyn.
The family has requested that in lieu of flowers, in honor of her memory, donations in Iris' name may be made to Riley Children's Hospital of Indianapolis, 500 N. Meridian St. #100, Indianapolis, IN, 46204, whose incredible research, doctors, and staff were instrumental in Iris' care.
