He was predeceased by: his parents, Donald Ballard and Delores Ballard. He is survived by: his wife Debra Ballard (Lewis); his daughters, Cassandra Brubaker (Monte) and Vanessa Ballard; his grandchildren, Kinleigh Pearl and Alexander Thomas; and his siblings, Bishop Randy Ballard (Annette) and Candace Balzano (Robert). He is also survived by many nieces and nephews.
