He was predeceased by: his parents, Willard Cook and Blanche Cook (Hartley); his wife Anna Marie Cook (Payne); his brothers, Charles Cook and William Cook; and his half-siblings, Dorothy Cortright, Harriet Pence, Helen Burris, Loretta Penrod and Harry Blizzard.

He is survived by: his children, Michael Allan Cook (Linda M.) of Martinsville and Deborah Ann Runyon (Brad J.) of Hartford City; his grandchildren, Adam Runyon, Allan Cook, Chase Runyon (Tiera Reynolds) and Dawn Gilmour (Damon); and his great grandchildren, Kinley Runyon, Alice Meyer, Daisey Gilmour, Jett Runyon and Dalilah Gilmour. He is also survived by several nieces and nephews.
