With heavy hearts, we announce the death of Dale J. Kozlowski of Mishawaka, Indiana, born in South Bend, Indiana, who passed away on September 14, 2023 at the age of 79. Family and friends are welcome to leave their condolences on this memorial page and share them with the family.

He was predeceased by: his parents, Joseph Kozlowski and Esther Kozlowski (Wanrzyniak); and his wives, Margaret Kozlowski (Labis) and Heidi Galloway. He is survived by: his stepchildren, Brenda Adams (Robert Lowry), Matthew Hertel (Melisa) and Jason Hertel (Amanda); his grandchildren, Derrick Adams, Ryan Adams (Melissa), Laine, Lilli, Louisa, Matheson, Michael Hertel, Jordan, Brittany Brown, Aidan, Riley and Evan Hertel; his great grandchildren, Dominic Quinlen, Wesley, Ella and Sophie Adams; and his sister Sharon Sanders.

Memorial contributions can be made in Dale's memory to Riverbend Cancer Services.
