He was predeceased by: his father Joseph Alvin Taylor; his brother Joseph Allen Taylor; and his stepfather Carlos Miller. He is survived by: his friend SxS; his mother Naomi Chadd Miller (Chadd) of Monrovia, Indiana; his siblings, Barbara Taylor Huntrods (Richard) of Camby, Kathy Taylor Reed (Charles) of Fort Pierce, Florida and Rick Miller (Tina) of Avon, Indiana; and his siblings' children, Suzanne Shields Edwards (Chad), Jim Shields (Kami) of Monrovia, Indiana, Renea Marsh Johnson (Ryan) of Avon, Jo Ann Taylor of Mooresville, Ryan Marsh (Angie) of Monrovia and Chadd Wallace of Fort Pierce, Florida. He is also survived by several extended family members and friends.
In lieu of flowers, memorial contributions may be made in Jack's memory to the American Cancer Society.
