He is survived by: his pet Tootsie; his sons, John "Jay" Perz, Jr. (Ashley) and Jordan Perz (Vannessa); his granddaughter Kassidee Lynn Perz; his sisters, Ramona Hall (Nick) of Batavia, Carole Zoven (Steve) of Lakeview and Frances Stevens of Rockford; and his former wife Lynn Sullivan. He is also survived by numerous nieces and nephews.
