She was predeceased by: her parents, Paul Phillips and Luetta Phillips (Acheson); her husband Gregory R. Horn; her siblings, Kenneth Phillips and Barbara Russell; and her niece Terry Woolridge.
She is survived by: her daughters, Stephanie Cottrell (Robert) of Monticello, Michelle Castro of Monticello and Jessica Small (Josh) of Lafayette; her brother Don Phillips (Sharon) of Lafayette; her grandchildren, Bayli Cottrell (Lexi) of Monticello, Kevyn Castro of Monticello, Ellen Cottrell of Carmel, Jada Butler (Kyler) of Lafayette and Mirna Lopez of Lafayette; her great grandchildren, Maleni Castro of Lafayette, Taylor and Jordan Cottrell of Monticello; her nieces, Shari Riera (Marty) of Lafayette and Jodi Eversole (Scott) of Omaha; her close friends, Denise Caldwell and Connie Tincher; and her pet Choppy. She is also survived by several other nieces and nephews.
