"Judy" Perkins; his grandchildren, Karisa Maiden (Kyle Frakes) of Winamac, Eric Maiden (Lisa) of Grandview and Rebekah Maiden of Monticello, Indiana.; his adoptive father Tom (Judy); his grandchildren, Amber Hook (Cody) of Monticello, Brooke (Ethan Glascock) of Monticello, Cale Maiden of Monticello, Samantha Maiden (Cade Baer) of Monticello, Tamra Maiden of Monticello, Ashley Chambers of Fortville, David, Roy Chambers and Tom; his great grandchildren, Maddy, Judson, Haisley Hook and Xander Maiden; his daughter-in-law Jody Maiden; his siblings, Ron Maiden (Liz) of Pittsboro and Martha Randel (Larry) of Zionsville, Indiana.; his son Jeff Maiden; his brother John Maiden; his sisters-in-law, Beth Maiden and Susie Maiden; his parents-in-law, Lloyd and Juanita Perkins; and his sister-in-law Janis Brock (Bob).
