He was predeceased by: his parents, Vernon Cook and Florence Cook (Clouse); his wife Beverly Cook; and his brother Morton Cook (Sonny). He is survived by: his sons, Richard Cook (Cathy) and Greg Cook; his grandchildren, Cameron and Carter Cook; and his brother Paul Cook. He is also survived by several nieces and nephews.
