He was predeceased by: his parents, Homer Cook and Helen Cook (Lineback); his former wives, Gloria Cook (Tersini) and Rebecca Cook (Smith); and his brother Marty Cook.
He is survived by: his wife Carol Cook (Jackson); his daughters, Renee and Michelle Cook; his siblings, Peggy Lobdell of Colorado and Tim Cook (Trudy) of Alaska; his stepdaughter Molly Howard (Cameron); and his granddaughters, Lila and Maggie Howard.
