He is survived by: his wife Fay Cook; his children, Chris Cook, Sondra Cook and Nathaniel Cook; his grandchildren, Lydia Cook and Gunnar Martin; his mother Shirley Zwicker; his siblings, Donnie Cook and Donna Metcalf; and his stepmother Patricia Cook. He is also survived by his many nieces and nephews.
