He is survived by: his parents, Richard Slack and Patricia Caldwell Slack; his brother Ryan Slack (Dawn); his aunt Reene Ann Slack; his uncles, David Caldwell (Marti) and Chuck Caldwell (Peggy); his cousins, Sara Dick (Larry), Nathan Caldwell, Tim Caldwell and Ian Caldwell (Patricia); Ed McConnell.
