He was predeceased by: his parents, Harry Dean and Carol Donnabelle Smith (Zellinger); and his brother James Lee Smith. He is survived by: his significant other Carol Diane Smith (Azbell); his children, Nicole Marie, Clarissa "Star" Cottey and Ricky Smith; and his sibling's children, Joe Smith (Brenda), Marti Eberson (Doug Brown) and Joy Davis (Michael).
Visitation was held on Tuesday, December 2nd 2025 from 4:00 PM to 7:00 PM at the Yoder-Culp Funeral Home (1911 S Main St, Goshen, IN 46526).
