He was predeceased by: his brother Ricky Mullendore. He is survived by: his daughter Sara Mullendore (David Hall); his grandchildren, William Cody Davis and Taylor Lynn Wood; his siblings, Melony Cline (Jim) and Jon Mullendore (Terrye); and his nephews, Christopher and Dalton.

Memorial donations may be given in his name to the donor's choice of charity.
