He is survived by: his caregiver Bonnie Simon (Dale) of Bennington, Indiana; his son Shane Carmack of Carrollton, Kentucky; his half-brother Keith Carmack of Carrollton, Kentucky; his stepgrandmother Pauline Searcy Cutshaw Poindexter of Carrollton, Kentucky; his stepbrother Michael Lee Cutshaw, Jr. of Carrollton, Kentucky; his uncles, Billy Harlow of Milton and Ronnie Carmack of Carrollton, Kentucky; his aunt Vicki Harlow of Carrollton, Kentucky; and his cousins, Angie Haskell (Eddie), Michael Harlow, Kristi Loveless (David), Doug Smith, Amy Turner and Daniel Carmack.
