He was predeceased by: his parents, Raymond E. Frye and Virginia Frye (Replogle); his wife Carol Ann Shearer; his son Forrest Wayne Small Jr.; and his siblings, Raymond Small, Jr., Virginia, Nancy and Cheryl.
He is survived by: his daughters, Tina Logan (Steve) of Star City, IN and E. Michelle Krock of Logansport; his brother Morris Small Sr. (Linda) of Logansport; his grandchildren, Jamie Rife (Jon), Tiffany Lambert (Derrick), Kayla Denny (Duncan Spicer), Ryleigh Krock, Cearra Snyder (Braden), Tabitha Small, Derrick Krock (Savannah), Jeremiah Small and Braxten Logan (Kourtney); and his close friend Sandra Bowyer of Peru.
