He was predeceased by: his father Jose Polio Perez. He is survived by: his mother Olga Perez (Rodriguez); his wife Milgian of Plymouth; his son Samuel Perez (Marlyn) of Mishawaka; his stepchildren, Kevin Rayes, Miguel Rayes, Jenny Rayes and Wilmer Orellana; and his siblings, Gonzalo Perez of Florida and Virginia Perez (German Morano).
