He is survived by: his mother Joan Anna Kulakowski (Jones) of Merrillville; his children, David Kulakowski of Lynwood, IL, Scott Candiano (Mandi Bridges) of Lake Station, IN, Catherine Gerber (Chuck) of Broken Arrow, OK, Danielle of Portage, IN and Alexander Rossman of Portage, IN; his siblings, Ernest Kulakowski Jr. (Marco Sampaolo) of Chicago, IL, John Kulakowski (Vicki) of Merrillville, IN, Teresa Cox (Pat) of Hobart, IN and Eileen Martinez; his grandchildren, Kelsey, Chase, Addison, Payton, Gradin, Chevelle, Landon, Alanah, Evan, Eryn and Bailey; and his siblings-in-law, Terri Ensign (Don) of Hebron, IN, Jamie Walter (Jeff) of Channahon, IL, Bob Smith (Sandy) of Chesterton, IN and Scott (Sherrie) of East Bremerton, WA. He is also survived by several nieces and nephews.
