He is survived by: his parents, Kenneth R. Ellsworth and Melinda S. Henry (Dennis Weimer); his daughter Makayla; his girlfriend Sara Klingenberger; his girlfriend's children, Lily, Gabe, Tempe and Max; and his grandmother Nola. He is also survived by 4 aunts; 6 uncles; many cousins; and countless friends.
