He is survived by: his wife Donna Shaw; his children, Michelle Yates (Michael) of Colorado, Tony Flack (Wendi) and Tracey Flack (Tammi); his siblings, Barbara Staker, Mark Adams (Sandi) and Jerry Adams (Jennifer); and his grandchildren, Justin Moore, Kyle Moore, Nathan Moore, Kyle Cupp, Traci Smith (Matthew), Logan Giroux (Braden), Mady Flack and Tony Keene. He is also survived by eight great grandchildren.
